New Patients
We are accepting new patients!
Two options are outlined below for establishing new patient care with Cornerstone Ob-Gyn. Select the option that best works for you.
OPTION 1:
- Fill out the online form by clicking here. A staff member will call you within two business days to confirm a date and time.
- Please arrive at least 15 minutes early incase your are asked to complete any additional paperwork that is relevant to your medical condition or concern.
OPTION 2:
- Call to schedule an appointment: 806.319.5211
- Download, print, and complete the forms below prior to your appointment.
- Please bring the completed forms with you to your scheduled appointment. It is important that you arrive at least 15 minutes early incase your are asked to complete any additional paperwork that is relevant to your medical condition or concern.
WHAT TO BRING TO YOUR APPOINTMENT
- Completed new patient forms if you chose Option 2
- Photo ID
- Current insurance card
- A list of current medications
- Medical records that might be useful
